Burning while passing urine in women: is it always a UTI?

Burning urination is often treatable, but the correct treatment depends on whether the cause is urinary, vaginal or related to another condition.

English version

Is burning urination always a UTI?

Burning or pain while passing urine is commonly associated with a urinary tract infection (UTI), but infection is not the only possible explanation. Vaginal irritation, insufficient hydration, certain products, sexually transmitted infections and other urinary conditions can produce similar symptoms.

Repeatedly taking antibiotics without confirming the likely cause may delay appropriate treatment and contribute to antibiotic resistance.

What symptoms may occur with a bladder infection?

  • Burning or pain while passing urine.
  • A frequent or urgent need to urinate, often passing only a small amount.
  • Lower-abdominal discomfort.
  • Cloudy, bloody or unusually strong-smelling urine.

Bacteria are the most common cause of bladder infections. Women are affected more often partly because the female urethra is shorter and located closer to the anus.

What else can cause burning?

  • Irritation from perfumed intimate washes, soaps or deodorants.
  • Vaginal dryness, including during breastfeeding or menopause.
  • A vaginal, vulval or sexually transmitted infection.
  • Dehydration or very concentrated urine.
  • Urinary stones or irritation following sexual intercourse.
  • Less commonly, bladder-pain conditions or other urinary disorders.

Burning with vaginal itching, unusual discharge, sores or pain during sex may require a gynaecological examination or infection testing rather than treatment based only on urinary symptoms.

When should you arrange a consultation?

  • Burning continues, worsens or returns soon after treatment.
  • You need to urinate frequently or urgently, or have repeated episodes.
  • You notice blood in your urine or have lower-abdominal or pelvic pain.
  • You also have unusual vaginal discharge, itching or sores.
  • Symptoms began after unprotected sex or with a new sexual partner.
  • You have diabetes, kidney disease or reduced immunity.
  • You are pregnant or may be pregnant.

Depending on your symptoms, a doctor may advise a urine routine test, urine culture, pregnancy test, vaginal or cervical testing, or selected blood tests or ultrasound. Not every woman needs every test. A urine culture can be particularly useful in pregnancy, recurrent infection, unusual symptoms or when treatment has not worked.

What can you do meanwhile?

Drink enough water unless a doctor has restricted fluids because of a heart, kidney or other condition. Avoid perfumed intimate products and do not douche inside the vagina. Do not use leftover antibiotics or another person’s prescription. Cranberry products do not treat an existing bladder infection.

Seek urgent or prompt in-person medical care

If you have fever or chills; pain in the back, side or groin; nausea, repeated vomiting or severe illness; visible blood in urine; inability to pass urine; urinary symptoms with fever or abdominal pain during pregnancy; confusion, faintness, breathlessness or unusual drowsiness; or symptoms with severe pelvic pain or heavy vaginal bleeding.
