English version
Pelvic organ prolapse occurs when one or more pelvic organs—such as the uterus, bladder or rectum—descend and press into or through the vagina because their supporting muscles and tissues have weakened.
Prolapse is common, particularly after childbirth and around or after menopause. It is not usually life-threatening, but symptoms can affect comfort, bladder or bowel function, exercise and sexual activity.
What symptoms can prolapse cause?
- A bulge, lump or something “coming down” in or outside the vagina.
- Heaviness, dragging or pressure in the pelvis.
- Symptoms that worsen after standing, lifting or later in the day.
- Difficulty emptying the bladder or bowel.
- Frequent or urgent urination, or urine leakage.
- Constipation or needing to press near the vagina to pass stool.
- Lower-back discomfort.
- Discomfort or altered sensation during sexual intercourse.
Some women have prolapse on examination but few or no symptoms. A vaginal bulge can also have other causes, so examination is needed to confirm what is present.
Why can it happen?
Factors that may contribute include pregnancy and vaginal childbirth, ageing and menopause, repeated heavy lifting, long-term constipation and straining, chronic cough, obesity, previous pelvic surgery and inherited connective-tissue weakness. Prolapse is not caused by poor hygiene or personal failure.
How is prolapse assessed?
A gynaecologist will ask about the bulge, pressure, bladder, bowel and sexual symptoms and how they affect daily life. A pelvic examination, sometimes while standing or bearing down, can identify which organ is involved and the degree of support loss. Urine testing, bladder assessment or imaging is used only when indicated.
Does every prolapse need surgery?
No. If symptoms are mild, observation may be reasonable. Treatment may include managing constipation or cough, adjusting heavy lifting, weight management when appropriate, and supervised pelvic-floor muscle training. A vaginal pessary—a removable support device—may relieve symptoms and can be useful for women who want to avoid or delay surgery.
Surgery may be considered when symptoms remain troublesome despite conservative care or the prolapse is more advanced. The procedure depends on the organs involved, previous surgery, general health, sexual activity and future pregnancy plans. A hysterectomy is not automatically required for every uterine prolapse.
When should you arrange a consultation?
- You see or feel a vaginal bulge.
- Pelvic heaviness or dragging persists or worsens.
- You have difficulty emptying the bladder or bowel.
- Urine leakage, urgency or recurrent infections occur.
- Symptoms affect walking, exercise, work, sleep or sexual activity.
- The exposed tissue becomes sore, dry or bleeds.
- You are pregnant, recently gave birth or hope to become pregnant.
If you cannot pass urine; severe or rapidly worsening pelvic or abdominal pain develops; a vaginal bulge becomes very painful, dark, swollen or cannot be gently reduced; there is heavy bleeding, fever, vomiting or foul-smelling discharge; or you are pregnant and have a new bulge, pain, bleeding or leaking fluid.
Pelvic organ prolapse is treatable. The most appropriate plan should reflect the symptoms, examination findings and your personal priorities—not the examination grade alone.
