WOMEN’S HEALTH JOURNAL • FERTILITY

Trying to conceive: how long should you wait before seeing a fertility specialist?

The right time to seek advice depends on age, how long you have been trying and whether either partner has a known fertility concern.

English version

Pregnancy does not always happen immediately, even when there is no known health problem. However, waiting too long for advice may not be appropriate—particularly when the woman is over 35 or either partner has a condition that could affect fertility.

The World Health Organization defines infertility as not achieving pregnancy after 12 months or more of regular unprotected sexual intercourse. Both female and male factors can contribute, so fertility assessment should normally involve both partners.

How long should you try before seeking help?

For couples with no known fertility concern:

  • Woman younger than 35: Arrange an evaluation after 12 months of regular unprotected intercourse without pregnancy.
  • Woman aged 35–40: Seek advice after six months of trying.
  • Woman older than 40: Consider an earlier or immediate fertility consultation rather than waiting for six or twelve months.

Age matters because both the number and quality of eggs decline over time. These timeframes are general guidance, not a requirement to wait when warning signs or known concerns are present.

When should you seek advice sooner?

For women, earlier assessment may be appropriate with:

  • Irregular, widely spaced or absent periods.
  • Very painful periods or known endometriosis.
  • Previous pelvic infection, reproductive-tract tuberculosis or sexually transmitted infection.
  • Previous ectopic pregnancy.
  • Known fibroids, uterine abnormalities or possible blocked fallopian tubes.
  • Previous ovarian or pelvic surgery.
  • Chemotherapy, radiotherapy or another treatment that may affect fertility.
  • More than one miscarriage or a condition that may reduce ovarian reserve.
  • Difficulty or pain during sexual intercourse.

For men, seek earlier advice with:

  • Previous testicular injury, surgery or undescended testis.
  • Sexual or ejaculation difficulties.
  • Previous chemotherapy or radiotherapy.
  • A known sperm problem or infertility with a previous partner.
  • Use of testosterone or anabolic steroids.
  • Significant testicular swelling, pain or a lump.

What does an initial fertility assessment involve?

An evaluation does not mean that IVF will automatically be advised. The first step is to understand whether ovulation, sperm, the fallopian tubes, uterus or another factor may be affecting conception.

  • Menstrual, pregnancy and medical history.
  • Review of how long and how frequently the couple has been trying.
  • Confirmation of ovulation where needed.
  • Pelvic ultrasound and selected hormone tests.
  • Semen analysis.
  • Testing of the fallopian tubes when indicated.
  • Screening for infection or other health conditions.

Not every couple needs every test. AMH or ovarian-reserve testing can provide specific information but cannot, by itself, confirm whether natural pregnancy will occur.

What can couples do while trying?

Regular unprotected intercourse every two to three days generally covers the fertile window without requiring perfect ovulation tracking. Taking folic acid before pregnancy, avoiding tobacco and recreational drugs, limiting alcohol and managing medical conditions are also important.

Avoid starting fertility hormones or unverified supplements without medical advice. Treatment depends on the cause, age, duration of trying and the couple’s preferences.

Seek urgent or prompt in-person care

If a pregnancy test is positive and there is severe or one-sided abdominal pain; pain occurs with vaginal bleeding, shoulder-tip pain, faintness or dizziness; there is sudden severe pelvic or testicular pain; pelvic pain occurs with fever, vomiting or foul-smelling discharge; vaginal bleeding is very heavy; or a testicular lump or persistent swelling is present.
